With a culture of "no-fault" issue tracking, I believe that we document patterns of behavior that isolate people like that. I have worked at too many places that punish post with the abuses of production and pre-production staff. By laying out expectations beforehand to ALL individuals involved in the productionline (including writers) and tracking results, over time you see who is not living up to their commitments.
No exceptions - deadlines and results involving me are also tracked. It feels like big brother at first but when everyone benefits from increases in productivity and living up to your commitments, you get buy-in. Or people leave.
And that's fine too.
